New Patients:Colorado Pain Clinic welcomes all new patients. Please contact us to schedule an appointment. Existing Patients: All existing patients are required to return on a monthly basis to re-evaluate their medications and individual treatment. CPC will not refill medications over the phone - each patient must be seen in the office for any medication refills. Things to bring for your appointment at Colorado Pain Clinic:Medication Refill Appointment: All patients who are on medications of which are prescribed from a provider at CPC is required to bring in all medications in their bottles to each visit at CPC. If you do not bring in your bottles of medications with you to your visit, you will be rescheduled and not seen at that time. Review of Films: All patients who are being seen in the office to review an ordered MRI, CT Scan or x-ray need to make sure that they bring in the films or CD with them to the appointment, or you will be rescheduled and not seen at that time. Colorado Pain Clinic cannot help you if you do not bring in the necessary information for the provider to work from - so it is in your best interest as the patient to make sure that the provider has all the correct and current information they need to in turn help you. ***In addition, if a patient is later than 15 minutes to their scheduled appointment, the patient will be rescheduled for another time. This helps Colorado Pain Clinic provide better customer service to all of our patients.*** |
